FOCUS ON...
Paul H. Sugarbaker, MD
The management of peritoneal metastases has expanded over the past 4 decades to become a new and prolific subdivision of surgical oncology. The original plan of management that employs cytoreductive surgery (CRS) to remove visible disease followed by perioperative intraperitoneal chemotherapy (HIPEC) to combat microscopic residual disease and preserve the surgical complete response remains intact. The requirement for a meticulous and complete CRS persists as an absolute necessity for long-term benefit. Treatment failure frequently documented on visceral peritoneal surfaces indicates the urgent need to improve HIPEC or find an alternative to HIPEC which is more effective. We have come a long way, we have a long way to go.
